All members start with a wellness visit. You will be receiving an email and text with intake forms for you to fill out.

Do not wait until the last minute to fill out your paperwork. Your provider will need 24 hours to view your information and prepare for your visit. We like to have knowledge about your health before your visit.

Your first visit is called a wellness visit, it includes a head to toe comprehensive exam.

We will be drawing labs on your first visit, please make sure you have only had water for the last 8 hours…12 hours would be optimal if you can do that. Drink plenty of water to plump up your veins.

If you have any health concerns, chronic conditions, would like to do a weight loss program, or start hormone therapy, this warrant additional labs, these are provided at an unbeatable cost for our members. We can discuss labs that can be added on during the visit.
