If this is your first visit:

You will be receiving an email and text with intake forms for you to fill out. Please check your junk folder.

Do not wait until the last minute to fill out your paperwork. Your provider will need 24 hours to view your information and prepare for your visit. We like to have knowledge about your health before your visit. If you do not complete your paperwork, you risk getting rescheduled.

For Direct Primary Care Members:

Your first visit is called a wellness visit, it includes a head to toe comprehensive exam, this is what you will be scheduling.

We will be drawing labs on your first visit, please make sure you have only had water for the last 8 hours, 12 hours would be optimal if you can do that.

Drink plenty of water to plump up your veins.

For Insurance based or cash pay patients:

If you have not been to Daisy in 2 years, you will be considered a new patient. If you have been, you will be considered an established patient. We offer in person and video appointments. If you have a high deductible, you may have to pay upfront for the visit, if we get reimbursed, we will refund you or give you a credit (your choice).

If you have any health concerns, chronic conditions, would like to do a weight loss program, or start hormone therapy, this warrants additional labs. We can discuss labs and cost during the visit.

Below are a few pointers to get you ready for your appointment.

All services must be cleared by the provider, as everyone will not qualify for our additional services. For example, if you have chronic kidney disease or congestive heart failure, there will be limitations.

Hormone Pellet Insertion: Do not schedule until you have been given the clearance by Angie. There is no need to fast…but make sure you are not taking aspirin, ibuprofen or drinking alcohol 3 days prior to your insertion. If you take a low dose aspirin (81 mg) for health purposes, that is fine, you can keep taking the low dose aspirin. Wear comfortable clothing, as the insertion is near the buttocks area.

Weight Loss: You have options when it comes to losing weight. It will depend on your commitment. The medications are designed to assist you on your journey, not do it all for you. You will need your labs drawn a few times during the treatment plans, but we will discuss and educate you. There is no contract, but we recommend 6 months of treatment. We offer semaglutide, tirzepatide, bella, contrave, meal planning. We can discuss which option is best for you. Injections: We can teach you how to do the shots, but some people rather not. Everyone must still be cleared, so you must be a current patient. If you are planning on asking for a shot during your appointment (b12 lipo, super b, glutathione, NAD), wear comfortable clothing, for us to access your buttocks if needed. We do not allow self scheduling appointments just for injections. So, a few ways you can go about this, ask for the shot during your visit, text us to see when we will be at your office, and we can tell you the best time to pop in (takes about 5-10 minutes). For vitamin d injections, these must be pre-planned, as the dosing goes off your labs. Now understand, the injections do not include a visit, so if you would like an assessment, and discuss your care with the provider, the cost is $75, as this is considered a follow up visit, and is best to just be done during a visit. If you would like us to do your weight loss injections, these are usually done on Fridays, and scheduled with you by your provider.

Vitamin Infusion: If you are coming in for a vitamin infusion, please wear clothing that will allow us access to your entire arm. Bring a book or your laptop. Some infusions are only about 30 minutes, some are up to 5-6 hours.

Ionic Foot Detox: If you are coming in for a foot detox. Please make sure your feet are clean prior to coming in. Sandals are best, as they are easy to slip off and on. Bring a book or your laptop. This is only done at our Glendale office.
