•Per Visit cost $150 - New or Established patients – Members $0 per visit

•Bella SR is $150 per month

Lab panel is $99 (we will accept your labs within 6 months, must include CBC, CMP, A1C, Lipid Panel).

These labs are included yearly for members. 

We do require a lab check at 3 months, 6 months, then every 6 months. 

Follow up lab panels are $50 for non-members and $20 for members.
