Below is a snippet of our new member set up: Also as an employer, we can come to you and do the wellness visit, and draw your employee labs once a year, we would just need a small room to use, we will even bring your employees brunch (because we know they will be hungry after labs). There is a yearly cost associated with this option, but it is a great way to assure your employees start off on the right track.

Welcome to Daisy! You get full access to our services, which means you begin with a full head to toe wellness visit and labs.

As a member, all of your visits (wellness, follow ups, sick visits) are included, which can be conducted in the clinic, by camera, or over the phone. Our focus is keeping you healthy.

We recommend all members to either have a catastrophic health insurance plan or be part of a health share, this will cover you if you need to have a major surgery, or if you get admitted to the hospital.

Our services work great with a high deductible plan or a health share plan. We have teamed up with Zion Health and they offer savings to our members who join, if you would like to join, let us know.

Shall you need anything else…we negotiated the cost for our members.

We have an in house pharmacy for your routine medications, if we cannot beat the retail pharmacy price, we can send your script to the pharmacy and send a goodrx coupon as well.

What to expect as a new member?

-As a member, some labs are included yearly. We will discuss, and do your labs on your first visit, so make sure you only have water 8-12 hours before your first visit. If you shall need any additional labs, we have a contracts for unbeatable prices! Included yearly is a CBC, CMP, Lipid Panel, and A1C.

-A follow up visit is conducted two weeks later to review your labs. When we do the follow up, the plan will include any concerns found, our recommended medications, supplements, etc and with goal(s) as well.

Schedule a morning visit, only water after midnight.

All new visits, must be in person.

You will receive your intake forms via email or text

(check your junk folder).

Please complete intake paperwork immediately, to allow your provider time to review.

If your forms are not completed at least 24 hours before your appointment, you risk getting rescheduled.
