We Cannot Schedule The COVID Vaccine over the phone
Please follow the steps below...get registered in 5-easy steps (read below).
You must register on the patient portal, create an account, then you can schedule.
We recommend that you schedule your first and second vaccine (28 days later) to secure your spots. You are able to schedule both in the portal.
Because of the large volume, you must register this way. You can register on a smartphone, tablet or computer, you can also have a loved one or friend help you.
If you are registering for the first time, it will send you a code to your email, or your cell phone.

-It will take you to the screen to set up your appointment. Nice and easy!

-Vaccine is for all adults 18 and over. You do not need to prove citizenship :)
-Vaccines are by appointment only.
-Vaccines will be administered via drive up, you remain in your car.
Drive around back. You will see a gate open, and a white tent.
-Please bring your photo ID, either an ID card, Driver's License, passport, any legit id that has your full name, date of birth and picture, like if you have a federal job, veteran id, etc.
-Please bring your insurance card
(if you do not have insurance, you are still eligible to receive the vaccine at no cost to you)
-You will have to remain on-site for 15-30 minutes after vaccination.
-Vaccines are not interchangeable, so if you had the Pfizer vaccine, we cannot administer a vaccine to you, as we have Moderna.
-Please be on time, we will be vaccinating many people in one day.
-If you are getting your second vaccine, please bring your vaccination card.
Preference is given to those getting their initial vaccine (please schedule your second vaccine as well, to secure your spot).
